TOURNAMENT REGISTRATION
Will be held on Friday, October 3, from 5-10 p.m., at:
Hawthorn Suites in Champaign
101 Trade Center Drive
Champaign, IL 61820

From I-57, take the I-72/University Avenue exit East (on University) to Neil Street. Turn right to the hotel.
From I-74, take the Neil Street exit South a few miles to the hotel.
The hotel is at the corner of Neil Street and Florida/Kirby Avenue.
You may also register on Saturday, October 4, from 7 a.m. - noon at the Main Site (Florida and Lincoln).
Teams must be registered 1 hour before their first game.
Teams playing their first game on Friday night MUST register at the Hawthorn Suites before their game.
Teams playing their first game at the Dodd's site must register on Friday night or at the main site on Saturday morning..
The following are required for registration:
Copy of your official, stamped state association Roster
2 Copies of the IYSA Tournament Roster
Copies of IYSA Medical Release Forms for EACH player
Player Cards (one laminated pass for each player)
Permit to Travel form (out-of-state teams only, from your state association)
Guest player forms (only if using guest players - limit of 4 per team)
****Non USYSA teams MUST fill out this form, and provide all documentation listed in the form.****
**You will be required to leave two (2) copies of your tournament roster, 1 copy of your state roster, guest player forms and IYSA medical release forms with us at registration. There will be NO copy machine available.**
